Fred is a mine of info; from what he said above it sounds like the sensor is faulty and the level it's sending to the ECU is either at its' max or min (12v / 5v, anywhere in between or 0v), depending what it sends when it thinks the coolant is too hot, hence the fan constantly running. Change the sensor.
